Parent Policy Agreement for photo project:
By signing below, I acknowledge, understand, and agree to the following if my dancer participates:

Voluntary Participation: I consent for my child to participate in a video/photo project.

Assumption of Risk: I understand that dance activities involve physical movement and carry a risk of injury (e.g., sprains, strains, fractures). I voluntarily assume these risks on behalf of my child.

Supervision & Responsibility: I understand my child will be supervised by DDH staff during all activities. However, I acknowledge that DDH Dance Academy is not liable for injuries occurring during participation.

Liability Waiver: I release and hold harmless DDH Dance Academy, its staff, instructors, and affiliates from any and all liability related to injury, property loss, or damages occurring during this event, to the fullest extent allowed by law.

Offsite Activity: I understand that the video project may include travel to a nearby off-site location. I give permission for my child to be transported or walk under staff supervision for this purpose.

Behavior Expectations: I understand that appropriate behavior and respect for instructors and fellow participants are expected. DDH Dance Academy reserves the right to contact a parent/guardian if a child needs to be picked up due to disruptive or unsafe behavior.

Personal Items: I acknowledge that DDH Dance Academy is not responsible for lost, damaged, or stolen personal belongings.

If your dancer(s) will not be attending the photoshoot or lock-in, you must notify DDH in writing before the Thursday of the Intensive. This allows us to finalize an accurate headcount of participating dancers. A permission slip will be provided at the start of the Intensive to confirm participation in these activities.
