By checking the box and signing below I acknowledge that for myself, and where applicable, my minor child(ren), acknowledge and understand that I/we am/are engaging and participating in an activity that involves modeling, physical activity, sport, dance, movement, acting, or general fitness that may also include the use of costumes, props and/or art supplies inside a studio, theater or outside area (herein referred to as “Activities”). I, for myself and where applicable, my minor child(ren) fully understand that my and/or my child’s participation in the Activities exposes me and/or my child to the risk of personal injury, illness and/or property damage. I hereby acknowledge that I and my child(ren) are voluntarily participating in these Activities and agree to assume all risks or injury that may result from such participation in any of these Activities.

I, for myself and where applicable, my minor child(ren), hereby release, discharge and agree not to sue Platinum Performing Arts, Inc., the corporation, its officers, directors, individual board members, or acting agents for any injury, loss or damage to my person or personal property that may occur arising out of, or in connection with, my and my child’s participation in any Platinum Performing Arts Activities, including the active or passive negligence of Platinum Performing Arts, Inc. or any other participant in the Activities. This waiver and release does not apply to the gross negligence or willful misconduct of Platinum Performing Arts, Inc. or its instructors, staff, officers, directors, or individual board members.

Medical Emergencies

With the above knowledge, I release and grant permission to Platinum Performing Arts, Inc. instructors, board members, employees, and volunteers to render temporary first aid to my child in the event of an injury or illness, and, if deemed necessary, to call emergency services for medical help.

Pick Up/Drop Off Policy

All Parents/Guardians:
Please treat the lobby as a quiet zone when any class is in session. If your child is young or needs you to stay on premises, sitting quietly out of view of the class helps avoid distractions and prevents students from getting nervous/anxious during class. There is absolutely no video/photos permitted while class is in progress, unless specifically stated otherwise by the instructor or Platinum Performing Arts Inc, employee. If you need to use the restroom during a class, please either go around to the back entrance or quietly walk around the students to avoid interrupting class. Parent interference during classes is very distracting and takes away from instruction time. Please only enter the studio space if an instructor has asked or if your child has a specific need for your assistance.
Students Ages 1-3:
A parent/guardian must stay on the premises for the duration of class and/or participate in the class.
Students Ages 3-6:
A parent/guardian may choose to stay on premises for the duration of class.
It is recommended that once the child is comfortable with attending class, parents transition to dropping them off a few minutes before class and picking them up at the end of class.
Parents/Guardians must walk into the studio to pick up the child so we know they are being brought home by a known individual and safely escorted through the parking lot.
Students Ages 7-13:
Students should be dropped off a few minutes before class and picked up at the end of class.

Students may, at the parent/guardian's discretion and consent, be released by a Platinum Performing Arts, Inc. employee to walk outside to the parking lot when the instructor/employee and the student both confirm arrival and recognition of the individual picking them up. If this recognition is not confirmed, the parent/guardian will be called to confirm identity or confirm approval for an alternate pickup person before the child is released from the premises. Platinum Performing Arts, Inc. must have written consent from the parent/guardian for this release pick-up method.

Photo/Video Release

By checking the box and signing below, I grant to Platinum Performing Arts, Inc. the absolute and irrevocable right and unrestricted permission concerning any photographs or video that he/she/it has taken or may take of me and/or my minor child named in which I or he/she may be included with others, to use, reuse, publish, and republish the photographs or video in whole or in part, individually or in connection with other material, in any and all media now or hereafter known, including the internet, and for any purpose whatsoever, specifically including illustration, promotion, art, editorial, advertising, and trade, without restriction as to alteration. I release and discharge Platinum Performing Arts, Inc. from any and all claims and demands that may arise out of or in connection with the use of the photographs, including without limitation any and all claims for libel or violation of any right of publicity or privacy. This authorization and release shall also inure to the benefit of the heirs, legal representatives, licensees, and assigns of Platinum Performing Arts, Inc., as well as the person(s) for whom he/she took the photographs. I am a legally competent adult and have the right to contract in my own name and/or child’s name. I have read this document and fully understand its contents. This release shall be binding upon me and my heirs, legal representatives, and assigns.
If for any reason I DO NOT grant permission to the Photo/Video Release and wish to deny Platinum Performing Arts, Inc. this right to use, I understand that it is my responsibility to notify Platinum Performing Arts, Inc. in writing via email (performplatinum@gmail.com) before attending classes. When a family does not agree to the Photo/Video Release, I understand that our account will be noted and therefore all staff/instructors will be made aware. I also understand that even when denying use, student(s) may have images/videos taken of them for choreography and class documentation purposes or in group settings. However, with written documentation of refusal of the Photo/Video Release, group photos that are posted to social media will not show the student's face of those under my account, whether by blurring, covering or cropping. Additionally, no images/videos of the students under my account will be included in any print or web marketing materials.

Bullying/Harassment Policy

Platinum Performing Arts, Inc. has a zero tolerance policy for bullying and harassment of any kind, including verbal, physical, online, and sexual. This policy applies to students, parents/guardians, clients, employees, board members, volunteers, and directors who participate at or are in any association with Platinum Performing Arts, Inc. Individuals who report bullying or harassment are protected from retaliation by Platinum Performing Arts, Inc. or any employee, board member, or person acting on the behalf of the company. Platinum Performing Arts, Inc. will promptly investigate complaints of bullying, harassment, or retaliation. Such investigations will be fair and impartial. All individuals associated with Platinum Performing Arts, Inc. are required to cooperate with investigations of bullying, harassment, or retaliation. If there is a confirmed case of harassment or bullying, disciplinary action will be taken, up to and including:
Immediate removal from any classes, production, or program the student is participating in
Immediate removal from any classes, production, or program the parent/guardian’s student is participating in
Immediate removal and/or ban from the premises
Termination
Legal action or support in legal action, if necessitated
