I, the undersigned (or parent/guardian if under 18) understand that attending rehearsal consistently is important for me (or my dancer) to not only learn and refine the choreography/steps, but for the cast and production as a whole. By checking this agreement, I acknowledge that I (or my dancer) have 3 or less rehearsal conflicts that include but are not limited to dance competitions, photo days, mandatory outside rehearsals, performances, already scheduled vacations, etc.
I understand that 3 absences is the maximum and all must be approved in advance. I understand that I should avoid scheduling additional vacations or events whenever possible during rehearsals times. I understand that excessive absences or missing partial rehearsals (over 3 rehearsals affected), no matter the reason, may result in a reduction of roles/stage time in future productions as well as in the current production if deemed overly excessive.

I, the undersigned, (or parent/guardian if under 18) do hereby waive any and all claims against the Greater Niagara Ballet Company, its Directors, Board Members, Employees, Volunteers and their heirs, executors and administrators, arising from any incident including, but not limited to, bodily injury, property damage, or loss sustained by me or the child in my care whom I am registering. By checking this agreement, I acknowledge that I have read and understand the foregoing statement and agree to its terms.

I, the undersigned, (or parent/guardian if under 18), do hereby agree that for promotional, publicity, advertising or commercial use, the Greater Niagara Ballet Company shall have the exclusive right to use photography, video recording, or other likeness of the performer who is registered on this form as desired and without additional consideration or compensation. By checking this agreement, I acknowledge that I have read and understand the foregoing statement and agree to its terms.

The following is a brief list of what will be expected of each dancer and their family and information that all involved in the production should be aware of prior to auditions:

1. Dancers must take it upon themselves to stay informed of any changes to rehearsals by checking the bulletin board, emails and remind messages.
2. Dancers must be able to attend all rehearsals. Dancers missing more than three (3) rehearsals may be dropped from the cast.
3. Dancers and their families are requested not to damage any property, to clean up after themselves and to respect the property of the theater, studio, waiting areas, and the Niagara Arts & Cultural Center at all times.
4. Dancers are expected to be on time and to not leave early for all rehearsals. Dancers who are repeatedly late or leave early to an extent deemed excessive may be dropped from the cast.
5. Depending on their ages, dancers may be required to help with the set up and strike of the show.
6. Each dancer and their family agree to participate in Greater Niagara Ballet Company's fundraising activities (i.e. selling program ads, helping at a fundraiser events, etc.)
7. Each dancer and their family is expected to volunteer to some degree during the rehearsal period and/or performances.
8. All dancers and families are expected to conduct themselves in a professional manner. Any actions by dancers or their families that are deemed discriminatory, harassment, retaliatory behavior, bullying, hostile, violent, or aimed at undermining or sabotaging the Company, the Company Mission, its Board, Directors, Professional Dancers, Student Dancers, Donors, Collaborators, Volunteers, Staff, or Assistants will not be accepted. Such behavior, if unable to be remedied, may lead to immediate removal from the cast, potential suspension from involvement in future productions or suspension of family members from being present in rehearsal, studio or performance spaces.
9. No requests for additional roles/parts or different roles/parts will be taken under consideration. Dancers/families may request less roles/parts.
10. Dancers who are no-shows to any rehearsals without notifying the Company will be dropped from the cast; future casting may also be affected. If you or your dancer decide for any reason not to participate after the cast list is released, we ask that you notify the Company right away so that roles can be recast in a timely manner. Emergencies that make notification difficult or impossible will be handled on a case-by-case basis.
11. Dancers and families should be aware that the Greater Niagara Ballet Company is a not-for-profit semi-professional company open to both student dancers ages 6 and up and professional dancers of all ages and, as such, may operate differently in terms of how productions are cast, number of roles/parts available, types of roles/parts available, etc. than for-profit dance studios.

I have read the above agreement and understand the rules. I understand that if I (or my child) damage any property of the studio of the Greater Niagara Ballet Company, the performance venue, or at the Niagara Arts & Cultural Center, I may be asked to pay reparation and leave the production. By checking this agreement, I acknowledge that I have read and understand the foregoing statements and agree to all terms.
