You understand you may only register your own children in which you are the legal guardian. All friends and family members must be separately registered by their legal guardians. This is so that all permission and liability/waivers are signed by a legal representative of the minor participating in activities at our facility. No Exceptions! Registration is open to ages 5-13 years only.

Minimum 3 registrations is required to host this event. If the event is canceled, BAD will credit your account for the total registration fee. This credit can be applied towards another event at BAD.

This event will be canceled if we receive less than 3 registrations. In the event that this event is canceled, we are likely to host Open Gym By the Hour that you may be interested in attending instead.

Payment for all events is due at time of registration. I understand that my credit card will go on file at B.A.D. for future payments that I may need to make. I authorize B.A.D. to charge my card for fees not paid before the time they are due.

You are fully aware and appreciate the risks, including the risk of catastrophic injury, paralysis and even death, as well as other damages and losses associated with participation in gymnastics/ninja activities and events. You agree that Twisters Inc. and the sponsor of any Twisters Inc. events, along with the employees, agents, officers, and directors of these organizations shall not be liable for any losses or damages occurring as a result of your child's participation in any event, except where such loss or damage is the result of the intentional or reckless conduct of one of the organizations or individuals identified above.

I, the parent and legal guardian, hereby give approval and permission for participation in all activities at the B.A.D. Incidental to such participation, I hereby waive, release, absolve, indemnify, and agree to hold harmless B.A.D., the employees, owners, supervisors, coaching instructors, and any subcontractors working with B.A.D. If my child has any physical condition that may impair their ability to engage in these activities, I will consult my physician prior to undertaking any physical exercise program. If necessary, I authorize B.A.D. to administer first aid and/or authorize medical treatment. Students are expected to carry their own medical and accident insurance. In signing, I agree to be responsible for any medical bills incurred during my child's participation at B.A.D. This waiver of liability, having been read thoroughly and understood completely, is signed voluntarily as to its content and intent. By signing this release, I understand the policies and liabilities that may occur in sports activities. I understand there are no refunds or credits given. In the event of a legal dispute, I will pay all B.A.D. legal fees. I certify that my child has no major disabilities or conditions that could impair his/her active participation. I am enrolling my child in classes that I believe they are physically and psychologically prepared for. This agreement is extended to cover all additional students and participants I enroll in any program at Twisters Inc. For any athlete who is not yet 18 years old: As legal parent or guardian of this athlete, I hereby verify by my electronic signature, that I fully understand and accept each of the above conditions for permitting my child to participate in classes, events, competitions, and activities conducted by B.A.D.

I give permission for B.A.D. to take and use any photos of my child for the purpose of advertising or website use, unless otherwise requested by me, in writing. I may submit a written document to keep on file requesting videos and pictures not be used.
