I am fully aware of and acknowledge the risks of participating in gymnastics, dance, cheer and/or tumbling activities for myself and my child(ren), that the risk is present by the nature of the activity, present when we are in the gym, and/or using any and all gymnastics-related equipment owned by Vitality Athletic Center, and that the risks include but are not limited to catastrophic injury, paralysis, and even death, as well as other damages and losses associated with participation. I agree that Vitality Athletic Center, along with employees, agents, officers, and directors of this organization shall not be liable for any losses or damages occurring as a result of my child(ren)'s participation in Vitality Athletic Center' programs or use of its facilities.

The undersigned, on behalf of the undersigned, and as the parent or legal guardian of the student(s) named above on behalf of such student(s), and in consideration of the opportunity to participate in the programs offered by Vitality Gymnastics, does hereby release and agree to indemnify and hold harmless Vitality Gymnastics, its shareholders, directors, officers, employees, agents, coaches, teachers and representatives from and against any and all liability, loss, damage, cost, expense, attorneys' fees, claim and/or cause of action, including, without limitation, any and all personal injury and property damage arising out of the use of equipment or participation in gymnastics, tumbling or other programs or competitions offered by or through Vitality Gymnastics. The undersigned further agrees not to cause any action at law or in equity to be brought on behalf of the undersigned or the student(s) named above on account of any of the foregoing matters.

I understand it is my responsibility to read understand the Vitality Athletic Center Handbook ("the Handbook") and that any violations of the policies, rules or expectations contained in the Handbook could result in disciplinary action, up to and including involuntary cancellation of membership. I also understand that I am to abide by any policies posted outside the Handbook.

I understand that I am to abide by the expectations explained to me in the Handbook and that unacceptable behaviors, blatant disregard of polices or disrespect of coaches will not be tolerated.

I am aware that the contents of the Handbook are presented as a matter of information and that except for the at-will provisions, the Handbook can be amended at any time. I understand and agree that the Handbook is for informational purposes only and is not intended to create a contract. I also understand that neither the Handbook nor any policy of the Vitality Athletic Center is a guarantee or promise of membership or continued membership.

If I have any questions regarding the interpretation or application of any policies or expectations contained in the Handbook, I should direct these questions to gym ownership.

Due to the new Safe Sport Policy's put in place by USAG it is required to have a consent form signed prior to photos being taken. By enrolling your child, as the parent or legal guardian of the gymnast being enrolled, you consent to photos to be taken at the facility as well as at any gym-related events and competitions. Photos may be used for promotional and marketing purposes by the gymnastics club.

A VALID credit card or bank draft is required for registration. $50 one time (non-refundable) fee is due upon registration to secure your reservation. The camp fee of $245 is due the first of the month. A 30 day notice is required to un-enroll for summer camp to receive a full refund (minus the $50 registration fee). Your camp fee is non-refundable 30 days prior to the camp start date. Cancellations are to be given via email to vitalitysummercamp@gmail.com
