I understand that payment is required in full at the time of enrollment for parents night out event. The full payment will be charged to my credit or bank account at the time enrollment to hold my students spot.

I understand If I choose to cancel prior to the event I will be deducted the 50% deposit and the remaining payment will be refunded. 50% deposit is non refundable and non transferable. I understand that if I choose to cancel after the event has started or not show up for the event on the day of the event all Fees are non refundable or transferable.

Parental Consent
And I, the minor's parent and/or legal guardian, understand the nature of the above referenced activities and the Minor's experience and capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from all liability, claims, demands, losses or damages on the minor's account caused or alleged to have been caused in whole or in part by negligence of the Releasees, or otherwise, including negligent rescue operations, and further agree that if, despite this release, I, the minor, or anyone on the minor's behalf makes a claim against any of the above Releasees, I WILL INDEMNIFY, SAVE AND HOLD HARMLESS each of the Releasees from any litigation expenses, attorney fees, loss liability, damage or cost any Releasee may incur as the result of any such claims.

Release of Liability
I hereby release, discharge, and covenant not to sue Park Elite Sports ,its respective administrators, directors, agents, officers, volunteers, and employees, other participants, any sponsors, advertisers, and, if applicable, owners and lessors of premises on which the Activity takes place, (each considered one of the "RELEASEES" herein) from all liability, claims, demands, losses or damages, on my account caused or alleged to be caused in whole or in part by the negligence of the RELEASEES or otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against any of the RELEASEES, I will indemnify, save, and hold harmless each of the RELEASEES from any loss, liability, damage, or cost, which any may incur as the result of such claim.

Medical Emergencies
Park Elite Sports has permission to render any necessary first aid emergency treatment to me or my minor child(ren) while in attendance at Park Elite Sports, and I hereby consent to any emergency treatment that may be required of me or my child(ren) by any licensed physician.

Photo Permission
Park Elite Sports Continuously photographs its activities and participants. These photos are used for advertising purposes in-house, online and in print, including but not limited to bulletin boards, newspaper, company website and Facebook. No personal information will be released at any time. I give Park Elite Sports my permission to use pictures of myself and or my child(ren) for said purposes. IF YOU DO NOT WISH TO GIVE PARK ELITE SPORTS PHOTO PERMISSION, REGISTRATION MUST OCCUR IN PERSON. THE ONLINE SITE CAN NOT PROCESS A 'NO' RESPONSE TO ANY OF THE PERMISSIONS.
