This Release, Indemnification, and Hold Harmless Agreement (“Agreement”) is executed in consideration for myself and/or my minor child(ren) being permitted to participate in CORE FRIDAY activities hosted by Combat Ministries, Inc., including its programs operating as Freedom Martial Arts & Fitness, Mentoring Warriors, and The Center by Combat Ministries, along with their owners, directors, officers, employees, agents, volunteers, insurers, representatives, and all persons or entities acting on their behalf (collectively, “Releasees”).

By signing below, I agree on behalf of myself, my minor child(ren), my heirs, assigns, personal representatives, and estate, as follows:

1. Acknowledgment of Activities and Risks
I understand that CORE FRIDAYS may include, but is not limited to:

Virtual Reality (VR) activities, including headset use, immersive simulations, motion-based experiences, and multi-player VR games

Basketball, Gaga Ball, 9-Square in the Air, and other physical games

Running, jumping, physical exertion, and participation in group recreational activities

Use of indoor facilities, equipment, technology, and common areas

I acknowledge that these activities involve inherent and unavoidable risks, including but not limited to:

Slips, trips, falls, collisions, or impacts with equipment or other participants

Dizziness, disorientation, nausea, motion sickness, or sensory overload related to VR use

Eye strain, headaches, or temporary vision disturbance

Sprains, strains, fractures, bruises, or other physical injuries

Aggravation of existing medical conditions

Emotional distress

Damage to clothing, glasses, phones, or other personal property

Risks caused by my own actions or inactions, the actions or inactions of other participants, or the negligence of Releasees

Risks that are unknown, unforeseeable, or not specifically listed

I understand that no amount of supervision, screening, or safety equipment can eliminate all risks associated with youth recreational activities.

2. Assumption of Risk
I voluntarily choose to participate or allow my minor child(ren) to participate in these activities.
I freely and expressly assume all risks, whether known or unknown, foreseeable or unforeseeable, including those arising from the negligence of Releasees.

If at any time I believe conditions are unsafe, or I or my child are unable to participate safely due to physical, medical, or emotional conditions, I will immediately discontinue participation.

3. Release and Waiver of Liability
To the fullest extent permitted by Indiana law, I hereby release, waive, and forever discharge the Releasees from any and all claims, demands, actions, causes of action, damages, losses, or liabilities of any kind arising out of or related to:

Participation in CORE Fridays

Use of VR equipment, sports equipment, or facilities

Transportation on or around the premises

Acts or omissions of other participants

The negligence of Releasees

This release does not apply to intentional misconduct.

4. Indemnification and Hold Harmless
I agree to indemnify, defend, and hold harmless the Releasees from any claims, demands, damages, attorney’s fees, or costs arising out of:

My participation or my minor child’s participation

My or my child’s actions or omissions

Claims brought by or on behalf of my minor child

Claims brought by other participants or third parties caused by my or my child’s conduct

5. Medical Fitness and Insurance
I certify that I and/or my minor child(ren):

Are physically and mentally able to participate safely

Have disclosed any relevant medical conditions

Are not experiencing symptoms that would make participation unsafe

I understand that Releasees do not provide medical insurance, and I agree that:

I have adequate insurance to cover injuries or damages, or

I will bear all costs of injury, treatment, or property damage personally

In the event of an emergency, I authorize Releasees to obtain medical treatment for myself or my child, and I accept full financial responsibility for such care.

6. Photo, Video, and Media Release
I understand that CORE FRIDAYS may involve photography or videography for program documentation or promotional purposes.
I grant permission for myself and/or my child to appear in such media unless I provide written notice opting out before the event.

7. Venue, Jurisdiction, and Governing Law
I agree that:

Any lawsuit arising from this Agreement shall be filed exclusively in a court of competent jurisdiction in the State of Indiana, and

Indiana substantive law shall govern this Agreement.

8. Severability
If any portion of this Agreement is found unenforceable, the remaining provisions shall remain in full force and effect.

9. Responsibility for Property Damage
I understand and agree that if my child or I cause damage to the facilities, equipment, VR systems, furnishings, or any other property belonging to Combat Ministries, Inc. or its partners, whether intentionally or through negligence, I may be held financially responsible for the reasonable cost of repair or replacement. I agree to make restitution upon request and acknowledge that failure to do so may result in collection efforts or legal action permitted under Indiana law.

10. registering for an event, camp, class, or activity through Combat Ministries / The Center, you are reserving a spot for your participant. Registration confirms your agreement to pay all associated fees for the selected program.

Payments are due according to the registration terms provided at sign-up. Spots are limited and staffing, supplies, and planning are based on registrations received.

Unless otherwise stated:

Event and camp fees are non-refundable.
Missed days or participant absences do not qualify for refunds or credits.
If payment arrangements are needed, they must be approved prior to the event or camp start date.
Failure to make payments may result in removal from the program and restriction from future registrations.

By completing registration, you acknowledge and agree to these payment terms.

11. Acknowledgment of Understanding
By signing below (electronically), I acknowledge that:

I have read and fully understand this Agreement.
I am giving up substantial legal rights, including the right to sue.
I am signing this Agreement freely and voluntarily.
Participation may not be permitted without this signed Agreement.
I may consult legal counsel before signing.
