We, the staff of Rising Star Gymnastics, recognize our obligation to make our students and their parents aware of the risks and hazards associated with the sport of gymnastics, tumbling and dance. Any activity involving height or motion creates the possibility of serious injury, paralysis and even death from landing on the neck, head and other parts of the body. Mats and pits do not eliminate this hazard.
1.With the above in mind, and being fully aware of the risks and possibility of injury involved, I consent to have my child or children participate in the programs offered by Rising Star Gymnastics including instruction, open workouts, running and conditioning, exhibitions, competitions, or clinics in which he or she may be participating or while traveling to or from any activity sponsored by Rising Star Gymnastics.
2. I/ We recognize that it is the responsibility of each participant to practice safe gymnastics and will instruct my/our child/children accordingly.
3. I/ We fully understand and will direct the minor participant that there are risks and dangers associated with participation in gymnastics events including but not limited to bodily injury, partial and/or total disability, viral infections, bacterial infections, other communicable diseases and illnesses, paralysis and death.
4. These risks may be caused by the negligence of the participant or negligence of others and there may be other risks not known to us at this time.
5. There may be other risks or other social and economic losses either not known to me or not readily foreseeable at this time.

I, my executors or other representatives, waive and release all rights and claims for damages that I or my child may have against Rising Star Gymnastics and/or it's representatives whether paid or volunteer.

I hereby give permission for my photographs of my child to be used by Rising Star Gymnastics in newsletters, advertising, print and video media, and the Rising Star Gymnastics website. I hereby release and discharge Rising Star Gymnastics from any and all claims arising out of the use of the photograph/video/sound that I or the minor child listed may have in this regard.

All campers must be picked no later than the camp ending time. Campers picked up 5-10 minutes late will be charged a $10 late pick up charge. Campers picked up more than 10 minutes late will be put into After Camp Club and parents will be charged the daily rate of $25.

*Both snack and lunch, which is to be provided by parents. If your child has no snack or lunch, we will provide them with food and charge the card on file.
*Sunscreen - Campers may put on sunscreen before they arrive or bring their own sunscreen to put on. Rising Star Staff will NOT apply sunscreen to any students.
*Warm clothes - it can be chilly outside.
*Refillable Water bottles - we have a water refilling station if needed
*Hair ties - Long hair should be pulled back with a hair tie to keep hair out of the face.
* Appropriate footwear - Woodchips from the park tend to get caught up in shoes such as flipflops and crocs.

I understand my child will go to the field behind the gym for snack, lunch and other outdoor activities when it is not raining. Full day campers may go to Peter J. Ferrante park if the park behind the gym is closed. Campers may put on sunscreen before they arrive or bring their own sunscreen to put on. Rising Star Staff will NOT apply sunscreen to any students.

No students may attend class if they are experiencing symptoms of illness including: Fever, chills, rapid breathing, shortness of breath, cough, runny nose/congestion, sneezing, extreme fatigue, sore throat, nausea or vomiting, diarrhea, headache, chills, new loss of taste or smell or any other symptoms of Covid, flu, or other virus.
