Release of Liability
I am enrolling my child in the family child care center operated by New College Child Center, Inc., located at 540 58th Street, Sarasota, Florida. I understand that the facility is leased from New College of Florida, and is located adjacent to the campus of New College of Florida. I acknowledge that there are risks of injury incident to attendance at a child care center. I assume all risks involved in participation in the child care center including risks to me, my family, and/or my child. I hereby release the State of Florida, the Board of Governors of the Florida State University System, New College of Florida, the New College of Florida Board of Trustees, the New College Foundation, Inc., their agents, officers, members, and employees, from any and all liability for any actions, causes of action, damages, claims, or demands which I or my heirs, executors, administrators, or assigns may have for any personal injuries, known or unknown, which may be incurred while participating in the family child care center. As parent or guardian of the enrolled child, I hereby consent to his/her participation in the child care center.
Payment Policies
Tuition is due on the first day of every month. By supplying my bank account or credit card details for secure storage via the Center's family portal, I authorize New College Child Center to initiate the appropriate tuition payment on the first day of every month. If I choose to opt out of automatic payments, I will inform the Center in writing and assume full responsibility for paying tuition in a timely manner. The Center strongly recommends use of its electronic payments system, and while it accepts non-electronic forms of payment such as cash, personal checks, and money orders, every payment not made electronically is subject to a $25 processing fee. Every payment submitted after the 10th of the month is subject to a $50 late fee. Medical Emergencies
I authorize staff in the child care program who are trained in the basics of first aid to give my child first aid when appropriate. I understand that every effort will be made to contact me in the event of an emergency requiring medical attention for my child. However, if I cannot be reached, I hereby authorize the program to secure necessary medical treatment for my child.
