Registration Agreement
I hereby acknowledge that all information entered through this portal for my child is accurate and up to date. I acknowledge that it is my responsibility to update contact information, i.e. persons authorized to pick up my child, phone numbers, et cetera as needed.
Personal Health Information
I hereby acknowledge that if my child has a health condition and I choose to not share that information with PCK Preschool & Junior Kindergarten, or I choose to not provide daily or emergency medication and applicable permission forms if need or prescribed, I agree to release the school and its personnel from any and all claim(s), which they now have or may hereafter have arising regarding the consequences of this choice. I am also aware that 911 may be called in an emergency.
Emergency Protocol Agreement
In the event of an emergency, I hereby grant PCK Preschool & Junior Kindergarten permission to provide any first aid deemed necessary. I give emergency personnel permission to transport my child to the hospital I have designated. If I or any other emergency contacts I've specified cannot be reached, the designated physician and/or hospital are hereby authorized to provide any emergency care deemed necessary for my child. I also authorize the transfer of my child's health records to the hospital. I hereby agree that I will be solely responsible for and will promptly pay any expenses that may be incurred by PCK Preschool & Junior Kindergarten in making emergency medical treatment available for my child.
Handbook Agreement
I hereby acknowledge that I have read the PCK Preschool & Junior Kindergarten Parent Handbook in its entirety and I will adhere to the policies held within.
Payment Policy
I understand that tuition will be posted on a monthly basis. Payments are due on the 1st day of the month, starting September 1st and continuing through May 1st of each school year. Any payments arriving after the 5th of the month will be assessed a 10% late fee. Any unpaid balances exceeding 30 days will result in student suspension until the balance is paid off. There is no guarantee there will be a seat in the class once the fees are paid.
