Emergency Medical Treatment Form
I understand a Emergency Medical Treatment Form must be completed and on file for each student.
Certificate of Immunization
I understand a current Certificate of Immunization signed by a licensed health care provider or the county health department must be on file for each student.
Enrichment and Registration Fees
I understand an Enrichment Program Fee of $100 per student is due with first month's tuition for Twos, Threes, Fours, and Kindergarten students.
I understand a one-time, non-refundable Registration Fee of $130 per student is due at time of registration for school year 2024-2025. Registration is not considered complete until registration fee is paid.
Tuition Payment Policy
I understand monthly tuition is due the first school day of each month and is late after the 5th day of each month. I understand a $30 fee for a returned check or declined credit/debit card will be applied to my family account.
Turn in your payment by check, money order, or cash in your Family Payment Envelope. Pay online using Parent Portal ~ we now accept Visa and Mastercard. A 4% convenience fee will be added separately to your family's account.
